Health Guarantee
Please print out, sign and fax or mail this form once your puppy has been reserved for you. Please call prior to faxing so the line will be ready for receiving.
A $250.00 non-refundable Deposit is required to reserve/purchase a puppy. No exceptions. Please read our health guarantee carefully. If you find that you cannot provide the essential care and love that a new baby would require, please do not adopt a puppy.
We, at The Strawberry Patch Kennels, love our work and more importantly, we love our dogs. Our "Mama's" and "Daddies" receive lots of love and attention along with all the necessary immunizations, health checks and overall hygiene care to keep them happy and healthy. In addition, we want our puppies to be healthy and happy companions for you and/or your family.
We want nothing but the best for our puppies. We believe a good vaccination program is essential, so we have consulted with one of the top veterinarians in the U.S. to help us give our puppies the best start they can possibly have.
We feed our puppies Eukanuba, Diamond or NutriSource small breed puppy food and we encourage the adoptive family (buyer) to have their puppy's particular favorite on hand when their new baby arrives.
Your puppy will come with a certificate of health, signed by a registered veterinarian, APRI or ACHC registration papers, AVID microchip and a current shot record.
We, the Seller, guarantee the puppy for 10 days from the date of purchase for life threatening diseases Parvo and Distemper. If a loss occurs during that time for these reasons, The seller will replace the puppy with one of equal value. A written statement from the veterinarian detailing the health problem is required from the buyer including copies of blood work results and notarized statements from the diagnosing veterinarian and a second, unassociated veterinarian, verifying the presence of the disease. The seller will not be responsible for any veterinarian fees or additional expenses for any reason.
We, the Seller, also guarantee the puppy for 1 year from date of purchase for life-threatening genetic disorders or genetic disorders that make him or her unsuitable as a companion/pet. Not considered life threatening or a disorder that would make your puppy unsuitable as a pet are: hypoglycemia* , sliding patella, open fontanel, un-descended testicle or bite. If your puppy is found to have a life-threatening genetic disorder or a genetic disorder making him or her unsuitable as a companion/pet, we would replace your puppy with one of equal value immediately or when a litter becomes available, or refund the full purchase price upon the return of the puppy at the buyer's expense. The seller requires a written statement from the veterinarian detailing the specific genetic condition and its impact on the puppy's life or quality of life. You, the Buyer, may choose to keep your puppy, thus assuming all past and future medical costs. We, the Seller, will not be responsible for any veterinarian costs incurred by the buyer at any time.
You must take your puppy to the vet for a wellness check within the first 72 hours of his or her arrival and provide him/her with the de-worming and vaccination schedule we provide to you. Your puppy has been on a well-maintained de-worming and vaccination schedule. If your veterinarian chooses to administer any additional vaccinations, our wellness guarantee is null and void. Puppies 12 weeks or older will have also received a rabies vaccination. No additional vaccinations are required until "booster" time.
You, the buyer, agree to provide clean housing suitable for his or her breed and protection from extreme heat or cold, adequate food, water and periodic veterinarian health care to your puppy. If, for any reason, you feel that you cannot provide these essentials....please do not adopt a puppy.
Our puppies are intended to be household pets and family companions. No guarantee is made as to disposition, conformation or breeding ability.
Seller does not guarantee "size" or "weight" of puppy when mature as there are numerous factors involved... including but not limited to puppy's diet and exercise. We can provide you with the "normal/average" size and weight of the breed and more specifically the puppy's parents so that you may make a general determination.
Buyer is not responsible for "change of heart" in Buyer or Change of address for Buyer where he or she is no longer allowed to have pets.
The Seller shall not be held responsible for acts of the puppy which are due to the Buyer's negligence.
Breeder is bound to this guarantee to the Puppy's Original Adoptive Parents. This guarantee is not transferable.
Buyer agrees to register puppy as "Crawford's Strawberry *******" (place a call-name of your choosing where the asterisks are located), within 6 months of adoption. Thank You, David & Kristen Crawford, The Strawberry Patch Kennels
*HYPOGLYCEMIA is not a disease or disorder but rather a condition caused by low blood sugar.
By sending your non-refundable $250.00 deposit and signing below you, the Buyer, agree that you have read, understood and agree to all stated herein. You agree to provide a safe, loving home to your puppy.
